Biliary obstruction symptoms vary widely among people with the condition, but are consistent. Most patients report a sudden onset of pain in the upper right quadrant of the abdomen, along with clay or acholic stools and dark urine. Other signs of biliary obstruction include hematochezia, right-sided abdominal pain, fever, weight loss, and intestinal malformations. The exact cause of this disorder is unknown, but it can result from an infection or malformation of the gallbladder.

The initial management of biliary obstruction depends on the patient's clinical condition and probable etiology. If the patient is febrile and is having fever, hospitalization may be required. If the biliary obstruction is caused by a stone, a liver biopsy will likely be necessary. Other treatments for biliary obstruction may include surgery to repair the blocked duct, antibiotics, and resection of the gallstones.

Early evaluation and management of biliary obstruction is critical. Surgical procedures may be indicated if the patient has multiple complications. An interprofessional care team should be consulted for a specialized assessment. If the biliary stent fails to remove all the stones, a resection will be necessary. If the patient's symptoms persist, endoscopic treatments can be considered. A ERCP is a diagnostic and therapeutic procedure in cases of biliary obstruction due to gall stones.

If noninvasive tests are unsuccessful, endoscopic procedures may be necessary. An ERCP, a needle aspiration, and EUS may be recommended. ERCP can also include biopsies and may be used for a diagnostic or therapeutic procedure. In some cases, endoscopic cholangiography (PTX) is an option for treatment.
