TRIGEN◊ SURESHOT◊ revolutionizes the distal locking of intramedullary nail osteosynthesis

The new magnetic field guided SURESHOT technology offers a high level of security and control during distal locking. The intramedullary nail can not only be locked without radiation, thereby relieving the OR team and patients. In addition, the system guarantees high precision, shortening the operating time and ease of use, even in extreme situations. SURESHOT has been available for the Smith & Nephew TRIGEN nail systems since spring 2010.

Intramedullary nail osteosyntheses are conventionally locked distally with the free-hand technique and are therefore always in the field of tension between radiation exposure and precision in screw placement. In addition to the high radiation exposure - the literature shows an exposure between 15 seconds and 2.7 minutes for the distal locking alone [1] - the total duration of the operation and sterility play a role: the positioning and swiveling of the image intensifier as well as fluoroscopy themselves significantly lengthen the procedure and impair sterility. Especially when locking the a / p screws on the distal lower leg, the extension position can interfere with fluoroscopy. In addition, incorrect drilling, broken drills, incorrect placement of screws or the residual rotation of the distal fragment are problematic.

Radiation-free locking technology

This can now be different with the magnetic field guided TRIGEN SURESHOT system. A receiving probe is inserted into the TRIGEN intramedullary nail. There is a transmitter on the drill sleeve that generates an electromagnetic field in order to determine the spatial position of the distal locking hole on the intramedullary nail. The SURESHOT system calculates the position and axis alignment of the drill to the hole. An interface connects the transmitter and receiver with one another. Its touchscreen surface guides the user through the steps, the monitor shows the axial alignment of the drill in 3-D in real time.

Convincing first results when using SURESHOT

The first clinical results are promising: A recently published study shows that SURESHOT ensures a high degree of precision and significantly reduces radiation exposure and the duration of the operation. In the case of tibial fractures, an average of 36 seconds of fluoroscopic fluoroscopy (equivalent to 0.785 rad) was saved, in the case of femoral fractures an average of 49 seconds (2.362 rad) were saved. For the surgeon and his team, with only one intramedullary nail osteosynthesis per week, this corresponds to a saving in radiation exposure of 6 hours over a period of 10 years. [2]

In the same study, SURESHOT achieved a very high hit rate of 100 percent on the tibia and 96 percent on the femur in the first attempt at drilling. The duration of distal locking on the tibia was reduced by 48 percent and on the femur by 32 percent.

The advantages of SURESHOT lie in the radiation-free, faster and safer procedure. In addition, the uncomplicated user guidance is also suitable for less experienced surgeons and can be a valuable support in extreme trauma surgical situations. For the patient, SURESHOT means shorter anesthesia and a lower risk of infection.

The new technology has been available for the TRIGEN nail systems since spring 2010. In the future, SURESHOT will be available for all Smith & Nephew nail systems.
